Charlton Athletic Community Trust (CACT) delivers a wide range of health improvement programmes across South-east London, from lifestyle interventions to primary care support.
To manage this scale of delivery, they needed a flexible digital solution that could coordinate multiple programmes, track health outcomes, and integrate seamlessly with other local systems and programmes.
SMG worked with CACT to implement a modular Health Improvement CRM, originally developed in partnership with the Royal Borough of Greenwich and widely adopted across public health services. The system supports:
The CRM provides secure, role-based access for practitioners, enabling them to manage patient cases, monitor follow-ups, and track health indicators including cholesterol, HbA1c, blood pressure, and mental health status. Its analytical capacity allows CACT to evaluate programme impact, generate actionable insights, and target resources where they are most needed.
During the COVID-19 pandemic, the platform demonstrated its scalability by supporting Greenwich’s borough-wide response. It processed over 65,000 engagements, 20,000 referrals, and coordinated more than 2,000 volunteers delivering essential services such as vaccination scheduling, food distribution, and mental health support.
Today, the Health Improvement CRM is a cornerstone of CACT’s work in community health improvement. It supports over 100,000 local people within the borough, providing the infrastructure for a large portion of all public health work in Greenwich. It enables staff to deliver tailored interventions, provides commissioners with robust outcome data, and ensures that the local community receives coordinated and effective support.
